Patient Membership Plan

Drs. Cusumano & Stuver are proud to promote a preventative dental relationship with our patients through our Patient Partnership Program. This program is designed to provide preventative services on an annual basis with exceptional discounts on all other dental services. The membership fee is $600 for adults 19 years and older and $550 for those 18 years and younger. This membership includes:

• 2 non-surgical preventative cleanings in a 12 month period
• 2 examinations in a 12 month period
• 2 fluoride treatment in a 12 month period
• 1 set of bitewing x-rays in a 12 month period (2 sets as needed for those 18 and younger)
• 15% off all other dental procedures (excluding Invisalign)
• Benefits are in effect for one year from date of membership

The Patient Partnership Program is renewable annually, based on the date of purchase. Membership is non-refundable, non-transferable and may not be used in conjunction with dental insurance or other discounts. Payment is required on the date of services rendered. All scheduled appointments require a 48 hour notice to reschedule in order to avoid a broken appointment charge of $50.
